The RCA approach is a major step forward in helping organisations improve their incident investigations. Health and safety management systems audits consistently reveal weaknesses in the incident investigation process which typically come from training deficiencies and a lack of effective support for the investigation process. More and more organisations recognise the importance of first-line leadership involvement in investigations, but few have effectively prepared these people for this vital role. First-line leadership cannot be expected to have the skills of a professional investigator, but they must have adequate skills to assure effective results.
RCA helps to meet this need in several key areas:
- It guides investigators through the key steps of the investigation process
- Its comprehensive listing of causes helps investigators ask the right kinds of questions and look for the right kinds of issues
- It assures that investigators will identify underlying or root causes
- It helps to link the causes of the incident to effective remedial actions
RCA is not solely for use by the less-experienced investigator. Major investigation teams find it extremely useful for many of the reasons cited above and they also find that in complex situations, it effectively helps keep track of the relationships between causes and the development of appropriate remedial actions.
Since experience is an important factor in determining the investigation results, the results of one particular investigation may not be reproducible from team to team. RCA has been used effectively in a variety of industries and can be readily understood by leaders at all levels.
This course looks to provide a practical approach to the process of implementing and managing events [accidents / incidents] in your organization.

All managers who are newly promoted recruited or have been in their job for some time but have no formal training. It is also suitable for appointed investigation staff looking to broaden their role with the organisation.
Workshop Objectives
Upon completion of the course, participants will be able to conduct an incident investigation through:
- Recognising and using the seven steps in the initial response to an accident
- Collecting evidence by utilising the five P's - position, people, parts, paper and process
- Applying guidelines for effective interviewing
- Applying the causation/problem solving model to analyse incidents
- Identifying losses, contacts, immediate causes, basic causes and lack of control in the incident
- Developing and categorising specific remedial actions
- Completing the appropriate information in an accident investigation report
- Applying the RCA chart and worksheet

- Trained and consulted on Risk management best practice for blue chip clients in Bahrain, Hong Kong, China, PNG, Indonesia, Canada, Turkey, Argentina, Chile, Peru, USA, Indonesia, Botswana, Zimbabwe, Kenya, South Africa, Saudi Arabia and Australia.
- Judged international Risk Management competitions in various sectors such as petro-chemical plants, power stations, motor manufacturing, mines, hospitals, hotels, forestry, and breweries.
- Held senior positions such as Principal Auditor, General Manager and Auditing Director responsible for auditing policy and protocols.
- Managed the accreditation of all HSE auditors for an international Risk Management company.
- Currently heads up a Technical Review Panel for HSE audit reports in SA and Australia.
- Has presented several papers at international conferences and conventions.
- Attained qualifications in General Management, Risk Management, and Auditor Certifications in ISO: 9001, ISO: 14001 and OHSAS: 18001.
